Which histories send a Wegovy plan to a specialist first, why each one matters, how urgent it is, and why most members need no referral at all.
Most members who start Wegovy never need to see a specialist. Your provider reviews your history, confirms you qualify, and writes the plan. A referral or consult is only added when something in your history means another service should weigh in before you start, or should stay involved while you are on the drug. When that happens, the referral is a written recommendation in your treatment plan, with the reason and the urgency spelled out, not a vague "see someone."
This article walks through the consults that come up most often, why each one matters with Wegovy specifically, and how urgent each one tends to be. Wegovy carries a short list of contraindications and a longer list of cautions on its label. [1] A contraindication means the drug should not be prescribed. A caution means it can be prescribed, but only with a plan for the specific risk. Some cautions are handled by your prescribing provider alone. Others are handled better with a specialist who already manages that part of your health, because they can adjust their own treatment while Wegovy is added.
Three things decide whether a consult is added: the service (who should be involved), the reason (what in your history triggers it), and the urgency (whether Wegovy waits for the consult, starts alongside it, or simply keeps the specialist informed). Your provider documents all three in the plan so you know exactly what is being asked and why.
Reason. Two separate histories point here. The first is thyroid. Wegovy carries a boxed warning for thyroid C-cell tumor risk, and a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2 is an absolute contraindication. [1] If your thyroid history is unclear, for example a relative with "thyroid cancer" of an unknown type, or a thyroid nodule that was never fully worked up, an endocrinologist can clarify what that history actually was before the drug is prescribed. The label states that routine calcitonin or thyroid ultrasound monitoring is of uncertain value, so the goal is to settle the history, not to run screening tests. [1]
The second is complex diabetes. If you take insulin or a sulfonylurea, adding Wegovy raises the risk of low blood sugar, because the label lists hypoglycemia with diabetes drugs as a serious reaction. [1] Those other medicines often need to be reduced as Wegovy takes effect, and the person who prescribes them should be the one adjusting them. [1] Co-management here is about coordinated dose changes, not about permission.
Urgency. An unresolved thyroid history is a stop until it is settled, because the contraindication is absolute. Diabetes co-management usually runs alongside starting, with the endocrinologist told before the first dose so the insulin or sulfonylurea plan is ready.
Reason. Pancreatitis is listed on the label both as a caution (a history or risk of it) and as a serious reaction. [1] If you have had pancreatitis, your provider wants to understand what caused it and whether that cause is still present before starting a drug that can trigger it again. Severe gastroparesis is the other trigger. Wegovy slows digestion, and in someone whose stomach already empties very slowly, the label lists ileus and obstruction among the serious reactions. [1] A gastroenterologist who already knows your gut can say whether the added slowing is acceptable.
Gallbladder disease, including stones and inflammation, is also a labeled serious reaction, but a prior gallbladder removal on its own does not usually require a referral. [1] That case is covered in taking Wegovy after gallbladder removal.
Urgency. A recent or unexplained pancreatitis episode generally means the consult happens before the first dose. A remote, fully explained episode is a provider judgment call and may only need the specialist informed. Severe gastroparesis is a before-starting consult.
Reason. Diabetic retinopathy is a labeled caution. [1] When blood sugar improves quickly, existing retinopathy can temporarily worsen, and Wegovy can produce that kind of rapid improvement in someone with diabetes. [1] If you have diabetes and either known retinopathy or no recent eye exam, an eye exam before starting gives a baseline, and follow-up exams during treatment catch any change early.
Urgency. Known retinopathy: exam before starting, then during treatment on a schedule the ophthalmologist sets. Diabetes with no known retinopathy: a baseline exam is the usual recommendation, and the provider decides whether it must come first. This consult applies to people with diabetes, not to everyone.
Reason. Renal impairment and volume depletion are both labeled cautions, and acute kidney injury is a labeled serious reaction. [1] The mechanism is usually dehydration: prolonged nausea, vomiting, or diarrhea drains fluid, and kidneys that are already impaired handle that badly. [1] For most members, a kidney check is a lab test, not a referral, and it sits inside the monitoring your provider does. A nephrology consult is reserved for established kidney disease where the specialist should know the drug is being added and can set limits.
Urgency. Established kidney disease: the nephrologist is informed before starting, and the provider decides whether to wait for a reply. Everyone with kidney impairment gets clear hydration and stop-rules, covered in normal effects versus stop-now warnings.
Reason. Wegovy for weight management is contraindicated in pregnancy, and the label says to stop at least 2 months before a planned pregnancy. [1] If you are planning a pregnancy, the plan needs a stop date and a conversation with whoever manages your reproductive care, so weight management and pregnancy timing are coordinated rather than colliding. Patients of childbearing potential are listed as a caution for the same reason, and contraception planning belongs in the same conversation. [1] If a pregnancy happens while on the drug, the label directs you to the Wegovy Pregnancy Registry at 1-877-390-2760 and your provider will stop the drug. [1]
Urgency. Planning pregnancy in the near term: Wegovy is usually not started, or a firm stop date is set at the outset. Longer horizon: start with a documented plan to stop at least 2 months before trying. [1] What happens after stopping is covered in stopping Wegovy.
Reason. Two situations. If your BMI is very high, or you have several weight-related conditions at once, an obesity medicine or bariatric program may offer options beyond a single drug, and your provider may recommend that evaluation alongside or instead of Wegovy. [2] If you have had prior bariatric surgery, the altered anatomy changes how the gut handles a drug that slows digestion, and the surgical team's input on absorption and dosing is valuable. The label is silent on a BMI threshold for this referral, so the provider sets it based on your history. [1]
Urgency. Rarely a stop. Usually runs in parallel, with Wegovy started if you qualify under the standard criteria in who qualifies for Wegovy. [1]
Reason. A drug that reduces appetite can make restrictive eating easier to hide and harder to stop. If your history includes an eating disorder, or your intake answers suggest disordered eating now, your provider will recommend an assessment before prescribing. This is not a judgment. It is a check that the treatment will not make an existing problem worse, and that someone is watching for it during treatment.
Urgency. Active disordered eating: assessment before starting. A remote, treated history: the provider decides, and may start with the mental health clinician informed.
Reason. Planned surgery is a labeled caution, and perioperative aspiration is a labeled serious reaction. [1] Because Wegovy slows stomach emptying, food can still be in the stomach at the time of anesthesia even after a normal fast, and that raises the risk of aspiration. [1] The label instructs you to tell your surgeon before any procedure. [1] This is the one consult that applies to nearly every member at some point, since it covers dental sedation, endoscopy, and elective surgery, not just major operations.
Urgency. As soon as a procedure is scheduled. The anesthesia team decides whether to hold the drug and for how long, and the label does not fix that interval, so follow their instruction. [1] If you stop for a procedure, restarting Wegovy after stopping explains how to get back on safely.
