Your first Wegovy dose is 0.25 mg, once a week, and it stays there for a full month. [1] That number is deliberately small. It is a settling-in dose, and the first month is about letting your stomach get used to a slower pace. What most people feel early on, a little nausea, fullness after small meals, some tiredness, usually eases within the first 2 to 4 weeks. [1]

Wegovy slows how quickly your stomach empties. [1] That is the point of the medicine, but your body needs time to adjust, and the lowest step gives you that time. In week one, expect to feel full sooner and to lose some interest in food. Mild nausea is common, especially after a larger meal, and some members notice burping, constipation, or looser stools. [1] These usually settle in the first 2 to 4 weeks. [1] If a step is hard to tolerate, your provider can hold you at it longer. [1] That shifts the calendar; it does not change the ladder. [2]
You do not need a special diet to start, but a few habits make the first weeks easier. Eat smaller meals, eat them slowly, and stop when you feel full rather than when the plate is empty. Greasy or very rich food sits heavily once digestion slows, so lean toward simpler meals with some protein while you adjust.

The Wegovy label does not set a rule about alcohol, so this is common sense rather than a protocol. [1] Alcohol is hard on a stomach that is already slower and more sensitive, it tends to make nausea worse, and it dehydrates you. If you drink, keep it small, eat first, and drink water alongside. The dehydration risk that matters most on the label is a different one: prolonged vomiting or diarrhea. [1] If either keeps going rather than passing, you lose fluid faster than you replace it, which also strains the kidneys. Wegovy carries a boxed warning about thyroid C-cell tumors, which is why it is not prescribed to anyone with a personal or family history of medullary thyroid carcinoma or with multiple endocrine neoplasia type 2. The label says routine calcitonin blood tests or thyroid ultrasounds are of uncertain value, so Wegovy does not come with a standing thyroid monitoring schedule. [1] What it does come with is a short list of symptoms to report right away: [1]
Other symptoms that mean message your provider promptly rather than waiting for your next check-in: severe or persistent abdominal pain, especially if it moves to your back (a pancreatitis signal), vomiting or diarrhea that will not stop, signs of an allergic reaction such as swelling of the face or throat, and, if you have diabetes, any new change in your vision. Wegovy for weight management is not used in pregnancy. [1] If you are planning a pregnancy, the label says to stop Wegovy at least 2 months beforehand. [1] If you could become pregnant, your provider will talk with you about contraception before you start; the label flags patients of childbearing potential as needing that conversation but does not set a single method, so the plan is set for your history. Because Wegovy slows digestion, it can change how quickly some oral medicines are absorbed. [1] That matters most for diabetes medicines, where low blood sugar becomes more likely when the two are combined. [1] Give your provider a full list of everything you take, including supplements, before your first dose. Do not combine Wegovy with another semaglutide product or with any other weight loss or blood sugar medicine unless it has been prescribed alongside it. [1]
Tell your surgeon and anesthesia team that you are on Wegovy before any planned procedure, even a minor one. Slower stomach emptying matters for anesthesia safety. [1] Whether to pause the medicine, and for how long, is a decision they make with your prescribing provider; the label lists planned surgery as a caution, not a fixed rule. [1]
At 4 weeks, you have finished step 1. The goal of this month is tolerance, not weight loss, and the label gives no 4-week number. [1] Some members see the scale move, many see appetite change first.

At 68 weeks, the trial figure is about 15 percent average body weight loss, and that word average matters.
