What a provider checks before your first Wegovy dose: weight, BMI, heart rate, glucose if diabetic, and the history questions that matter more than scans.
Before your first Wegovy dose, your provider checks a short list of things. Most of it is history, not testing. The Wegovy label names a few baseline items and a few situations that need care or rule Wegovy out entirely. [1] Everything beyond that short list is a provider decision made for your history, not a fixed requirement.

The Wegovy prescribing information lists two monitoring parameters: heart rate, and blood glucose at baseline if you have diabetes, then periodically. [1] That is the whole list of label-mandated measurements. It also names contraindications and cautions, which are history questions your provider must ask before prescribing. There is no label requirement for a panel of bloodwork, a thyroid scan, or imaging before you start. [1]
So the pre-start visit is mostly a conversation. A good one is thorough. A short one is not necessarily a bad one, as long as every item below has been covered.
What is checked: your current weight and height, used to calculate body mass index.
Why: BMI is how eligibility for Wegovy is defined, and it is the starting point every later check-in compares against. [1][2] Without a documented baseline there is no way to tell whether treatment is working or whether a dose step should be held or advanced.
When: before the first prescription. In a chat-based visit this is self-reported. Use a scale you will keep using, in the same conditions each time, so the trend is real and not a scale difference. The thresholds that qualify you are covered in the eligibility article.
What is checked: resting heart rate.
Why: heart rate is one of the two label monitoring parameters. [1] Semaglutide can raise resting heart rate in some people, and the only way to know whether a later reading is a change is to have a baseline. [1]
When: at baseline, then as part of ongoing monitoring. How often it is rechecked after that is set by your provider. What is checked: a blood glucose measurement, or an A1c, which reflects average glucose over recent months.
Why: the label requires a baseline glucose if you have diabetes, followed by periodic checks. [1] Semaglutide lowers glucose, and in someone already on diabetes medication that can tip into hypoglycemia. [1] The label lists hypoglycemia with diabetes drugs as a serious reaction. [1] A baseline tells your provider how much room there is and whether other diabetes medications need adjusting.
If you do not have diabetes: the label does not require it. [1] Some providers check it anyway, because a person seeking weight treatment often has undiagnosed prediabetes or diabetes, and finding it changes both the monitoring plan and the conversation about other medications. Whether to order it for you is your provider's call based on your history and risk factors, not a rule.
When: before the first dose if you have diabetes. Otherwise, if ordered, at a time your provider chooses.
What is checked: whether you or anyone in your family has had medullary thyroid carcinoma, or whether you have multiple endocrine neoplasia type 2, known as MEN 2.
Why: this is Wegovy's boxed warning. [1] Semaglutide caused thyroid C-cell tumors in rodents, and it is not known whether the same happens in people. [1] Because of that uncertainty, a personal or family history of medullary thyroid carcinoma, or MEN 2, is a contraindication. [1] If either applies to you, Wegovy is not prescribed. There is no workaround and no dose that makes it acceptable.
Why it is not a scan: the label states that routine monitoring with calcitonin blood tests or thyroid ultrasound is of uncertain value. [1] A test with uncertain value can produce a result nobody knows how to act on, which can lead to more testing and worry without a clear benefit. So the screen is the history question, asked directly and answered honestly. If you do not know your family history, say so, and your provider will decide how to proceed.
What you are told at the same time: report a neck mass, trouble swallowing, shortness of breath, or persistent hoarseness at any point during treatment. What is checked: whether you have ever had pancreatitis, and whether you have known risk factors for it.
Why: pancreatitis is listed as a serious reaction to semaglutide, and a history of it is a label caution. [1] The label does not say a past episode rules Wegovy out, but it does mean your provider weighs the risk more carefully and may choose a different treatment, or proceed with closer attention to abdominal symptoms. [1]
When: at the pre-start history. If you have had pancreatitis, expect a conversation about the cause, how long ago it was, and whether the cause is still present.
What is checked: whether you have gallstones, have had gallbladder inflammation, or have had your gallbladder removed.
Why: gallbladder disease, including gallstones and cholecystitis, is a listed serious reaction. [1] Rapid weight loss from any cause raises gallstone risk on its own, so the two effects stack. [1] Knowing your gallbladder history in advance helps your provider interpret right-sided abdominal pain later, rather than guessing. When: at the pre-start history. No routine gallbladder imaging is required by the label before starting. [1]
What is checked: whether you have known kidney disease, and whether you are at risk of dehydration. A blood test of kidney function may be ordered.
Why: acute kidney injury is a listed serious reaction, and the mechanism is usually indirect. [1] Nausea, vomiting, and diarrhea, which are the most common side effects, can lead to volume depletion, and a dehydrated kidney is a vulnerable kidney. [1] The label lists both renal impairment and volume depletion as cautions. [1] If your kidneys are already compromised, or you take medications that stress them, the margin is smaller.
When: the history is taken before starting. Whether a baseline kidney blood test is ordered depends on your history. It is not a label requirement for everyone, so your provider decides. [1] Reducing the GI effects that drive the risk is its own topic, covered in managing GLP-1 nausea.
What is checked: if you have diabetes, whether you have diabetic retinopathy, and when your last eye exam was.
Why: diabetic retinopathy is a label caution. [1] Rapid improvement in blood glucose can temporarily worsen existing retinopathy, so a provider needs to know it is there before glucose starts to fall. [1] This is a history question, and the answer often comes from your most recent eye exam rather than a new one.
When: at the pre-start history if you have diabetes. Whether an eye exam is needed before starting, or a referral to an eye specialist, is your provider's call. Situations that need a specialist first are covered in when Wegovy needs a specialist first.
What is checked: whether you are pregnant, breastfeeding, or planning a pregnancy, and whether you could become pregnant.
Why: pregnancy is a contraindication for Wegovy used for weight management. [1] Weight loss offers no benefit to a pregnancy and may harm the fetus. [1] Breastfeeding is a contraindication for the oral form. [1] Because semaglutide stays in the body for a long time after the last dose, the label says to stop Wegovy at least 2 months before a planned pregnancy. [1] If you become pregnant while on it, stop and tell your provider. There is a Wegovy Pregnancy Registry at 1-877-390-2760 that tracks outcomes. [1]
When: before starting, and again at every check-in. Plans change, and this is the one item that most often shifts during a year of treatment. If you are of childbearing potential, expect your provider to raise contraception as part of the conversation.
It helps to be clear about what the label does not require, because you may see it listed elsewhere as routine. No routine calcitonin. No thyroid ultrasound. No baseline liver panel, lipid panel, or imaging for everyone. [1] Some providers order some of these based on your history, and there can be good reasons. But if you are asked for a long list of tests as a condition of starting, it is fair to ask which ones the label requires and which ones are for you specifically.
