If you want a routine that genuinely slows visible aging, the part to get first is a prescription for tretinoin. Strength matters less than people assume. 0.05% is the most studied concentration and 0.025% is also effective with easier tolerability, and because response is dose dependent, a higher strength may act faster but tends to land in a similar place while causing more irritation. Niacinamide belongs here mainly where tretinoin is hard to tolerate. It supports the skin barrier, and the 12 week split face study found real if modest improvements. [7] Honest version: a cheap generic 5% niacinamide serum does the same job as an expensive one, and if your skin is settling in fine without it, you do not need to buy it. Vitamin C goes in the morning and tretinoin at night, separated by time of day for practical reasons rather than because they cancel each other out. The strongest human data for a topical vitamin C formula is about photoprotection, not wrinkle reduction. [8] If your budget only stretches to two products, make them tretinoin and sunscreen. Pigment and surface roughness move first, over roughly the first 4 to 10 months, while new dermal collagen takes about a year of continued use. [3][10] In a 10 month randomized trial of 0.1% tretinoin for sun related brown spots, 83% of treated patients had lightening of facial lesions against 29% on vehicle, and cleared lesions had not returned six months later. [4] Benefit depends on continuing, usually at a lower strength or reduced frequency once you have the result you wanted. [3] The first two months are the hard part. The label is blunt about this. Tretinoin emollient cream 0.05% has not demonstrated an effect on coarse or deep wrinkling, skin yellowing, lentigines, telangiectasia, skin laxity or dermal elastosis, and its effectiveness has not been established in people over 50 or in moderately to heavily pigmented skin. [1] A separate trial of a different product, 0.1% tretinoin cream, did lighten sun related brown spots. [4] Different strength, different study, so treat the label claim as the conservative one.
Those gaps are where an added ingredient can genuinely earn its place, which is different from adding it because it is trending. How tretinoin pairs with the newer anti aging ingredients sorts them by which have controlled trials and which have supplier claim data only.
