How to arrange the Shield colorectal cancer blood test online in the US: the clinician conversation, the order, the blood draw, costs and results.
Most of it, and the part you can do from your phone is the part that decides everything else. Shield is a blood test for colorectal cancer screening, and Guardant Health states that a healthcare provider has to order it. [1] The conversation that leads to that order, the check on whether Shield fits you, the cost questions and the result review can all happen by message. The blood draw itself cannot: a trained professional collects the sample at a clinic, a laboratory or your own address. [1]
So the realistic goal is not to buy Shield online. It is to use an online conversation to get the screening decision made, the order placed, the draw booked and the result handled without a waiting room. Here is how that works, step by step.
The FDA approved Shield for adults aged 45 and older at average risk of colorectal cancer who are due for screening. It is not a replacement for a colonoscopy that is investigating symptoms, or for surveillance colonoscopy in someone at high risk. [2]
Being eligible is not the same as Shield being the best choice. The American Cancer Society's 2026 guideline found that blood-based tests detect fewer stage I cancers and fewer advanced precancerous lesions than the established stool tests, and places them after the preferred tests, for people who decline or do not complete one. [3] If bowel preparation, time off work, stool collection or cost has stopped you screening so far, say so. That barrier is what a clinician needs to address, and it is also what makes Shield a reasonable choice for some people.

Symptoms change the question from screening to investigation, and a recent negative test can mean you are not due yet. Both are worth knowing before anyone orders anything. If your own clinician has not heard of Shield, Guardant publishes a patient starting page with an eligibility questionnaire and a fact sheet you can share. [4] Completing that questionnaire is information for the conversation, not an order. A website advertising online lab testing is not the same thing either, so ask directly whether a service can assess you and place a Shield order before you pay for anything.

This is the one step that happens in person. Guardant describes three places the sample can be collected: the ordering clinician's office, a nearby laboratory, or your own address, where a mobile phlebotomist arranged through Guardant's team comes to you. Guardant states there is no charge for the mobile visit itself. [1] A home visit means a professional draws the blood; Guardant advises against collecting it yourself. [1]
The sample goes into the Guardant Shield Blood Collection Kit, up to four tubes, and is sent to Guardant Health's laboratory for testing. [2] Before you book, confirm four things with whoever is collecting:
Shield needs no fasting and no special preparation. [1,2] If other tests are being collected at the same visit, follow their instructions instead. Guardant estimates results in approximately two weeks. [1]
Ask for an estimate of your own responsibility before the draw, with the clinician visit, the collection and the laboratory test listed separately. Confirmed as of 2026-09-08: Medicare's benefit page covers a blood-based colorectal screening test once every three years for people aged 45 to 85 who have no symptoms of colorectal disease and are at average risk, with nothing to pay when the provider accepts assignment. If that test is positive, Medicare also covers the follow-up colonoscopy as a screening test. [5]

A positive Shield result means a diagnostic colonoscopy is the next step. A negative result does not rule out colorectal cancer, and the FDA says people with a negative result should continue to take part in screening. [2]
Decide who will act on the result before the blood is drawn. If it is positive, the ordering clinician arranges the colonoscopy. If it is negative, ask when your next screening is due and which test it will be. Keep a copy of the report so any future clinician can see exactly which test you completed and when.
