What to Know Before Your First Finasteride Dose

The short version

Finasteride is the easiest medication routine in medicine: one 1 mg tablet, once a day, with or without food. [1] The hard part is entirely psychological: nothing visible happens for months, the first thing you may notice is more shedding if you started minoxidil alongside, and the medication only works for as long as you take it. Most men who quit finasteride quit in the dead zone between starting and seeing, which is precisely the period the drug is working invisibly. This is the briefing that should come with the bottle: the real timeline, the honest side effect numbers, and the three rules that never expire.

What it actually does

Pattern hair loss runs on dihydrotestosterone, DHT, a hormone made from testosterone that progressively shrinks genetically sensitive follicles until they produce wisps instead of hairs. Finasteride blocks the enzyme that makes most scalp DHT. Follicles stop shrinking, and some recover. [1][3] It is prevention first and regrowth second, which is why starting earlier beats starting later, and why the crown responds better than a long-bare temple. [3][4]

The timeline, month by month

• Months 1 to 3: nothing visible. The label says it plainly: daily use for three months or more is generally necessary before benefit is observed. [1] Hair grows about a centimeter a month, so even a follicle rescued on day one needs months to show it.
• Months 3 to 6: the trend starts. Less hair in the drain is usually the first real signal, and it is easy to miss without the baseline photos described in the follow-up article.
• Month 12: the verdict. Photos against baseline. In the trials, treated men held or gained while the placebo group kept losing, with visible improvement in a meaningful share and the best results at the crown. [4]

The honest side effect numbers

In the clinical trials, sexual side effects, lower libido, erectile difficulty, or reduced ejaculate volume, occurred in a small minority of men, on the order of a few in a hundred, and were somewhat more common than on placebo; they typically resolved on stopping, and often even with continued use. • Mood. Reports of depression and anxiety exist on this drug class. Any new or worsening low mood after starting is a stop-and-message event, not something to ride out. [5]
• Persistent symptoms after stopping. A condition called post-finasteride syndrome, persistent sexual or neuropsychiatric symptoms after discontinuation, is reported and debated: it lacks an established mechanism and remains a controversial diagnosis, while the on-treatment side effects themselves are well documented. • The PSA rule. Finasteride lowers PSA, the prostate cancer screening number. Tell any clinician ordering a PSA that you take it, so the result is interpreted correctly. [1]
• The handling rule. Women who are or may be pregnant must not handle crushed or broken tablets; the coating on an intact tablet prevents contact with the active ingredient. [1]
